Key takeaways
- Lilly raised its 2026 sales outlook to US$91–92 billion as Mounjaro and Zepbound drove growth. [1]
- Shares rose 6.73% in an 11:15 ET snapshot, not a closing price. [2]
- Headline adjusted EPS guidance fell because acquired-research charges outweighed an underlying upgrade. [1]
- For the wheel, assignment cash and the next competitor report matter more than chasing the rally. [5][8]

What happened: a stronger sales outlook, a complicated EPS number
Eli Lilly jumped after its October 29 third-quarter results. Yahoo Finance showed US$1,374.99 at 11:15:38 ET, up $86.69, or 6.73%, from the October 28 close of $1,288.30. These are dated regular-session observations, not live offers or the final daily result. All financial amounts below are U.S. dollars. [1][2]
The saved page recorded 2,132,171 shares traded, against its displayed average volume of 2,495,029. That is 85.46% of an average full day; it is not a time-adjusted measure of unusual volume. The session range at retrieval was $1,330.00–$1,408.38, showing that a strong opening reaction still left room for large swings. [2]
For the quarter ended September 30, revenue reached $24.397 billion, versus $17.601 billion a year earlier. Reported diluted earnings were $7.22 per share, up 16%; adjusted, or non-GAAP, EPS was $7.48, up 7%. Importantly, both current-quarter EPS measures still included $4.57 per share of acquired in-process research and development charges. Adjusted does not mean those charges disappeared. [1]

Why demand and pricing pulled in opposite directions
Lilly attributed the 39% sales increase to 53% volume growth, partly offset by 15% lower realized prices. Those are company-reported growth drivers, not percentages to add mechanically. More treatment demand can expand revenue even while the amount collected per unit falls. [1]
Mounjaro generated $10.723 billion, up 65%, and Zepbound contributed $5.352 billion, up 49%. Together they produced $16.075 billion, a calculated 65.9% of company revenue. That concentration helps explain both the growth opportunity and the sensitivity to reimbursement, pricing and competing diabetes and obesity medicines. [1]
| Measure | Previous outlook | Updated outlook |
|---|---|---|
| Revenue | US$85B–87B | US$91B–92B |
| Non-GAAP EPS | US$35.50–36.50 | US$35.00–36.00 |
| Performance margin | 49.0%–50.5% | 52.0%–52.5% |
| Non-GAAP tax rate | 18%–19% | 19%–20% |
The sales-guidance midpoint increased from $86 billion to $91.5 billion. Yet the adjusted EPS midpoint declined from $36.00 to $35.50: management raised underlying EPS guidance by $4.07, then included $4.57 of third-quarter acquired-research charges. The net change is minus $0.50. This is an accounting bridge, not evidence that all parts of the business weakened. [1]
A sales upgrade and an EPS downgrade can be true at the same time. [1]
There is a second qualification. Quarterly revenue included approximately $900 million of benefits, primarily Jardiance milestones and favorable rebate and discount-estimate adjustments. Jardiance milestones contributed $410 million, and Lilly says no further milestones remain. Treating every dollar of this quarter as a repeatable sales run rate would therefore miss an important limitation. [1]
- Demand: Mounjaro and Zepbound were the main growth engines, not an across-the-board price increase. [1]
- Pricing: Lilly cited lower Mounjaro realized prices outside the U.S., primarily from Chinese reimbursement-list inclusion. [1]
- Spending: acquired research can depress current earnings without establishing whether the purchased pipeline will ultimately succeed. [1]
Sector context: a peer checkpoint, not a blanket drug-stock call
Novo Nordisk’s U.S.-listed shares were also higher: $39.04, up $0.75 or 1.96%, at 11:15:29 ET. Its November 4 results for the first nine months of 2026 are the next dated peer checkpoint. Pricing, access and treatment demand deserve comparison; Lilly’s growth does not by itself establish Novo’s future sales or either company’s market share. [4][5]
The macro backdrop is not risk-free either. On October 28, the Federal Reserve held its policy range at 3.75%–4.00%, with two dissenters preferring a quarter-point increase. That provides interest-rate context for equity valuations. The evidence here does not isolate Fed policy as the cause of Lilly’s rally. [6]
What it means for option sellers
Lilly’s earnings release has passed, but that removes only one scheduled information event. OptionCharts displayed 30-day implied volatility of 34.55% and IV rank of 33.74%, dated October 29, when opened at 11:22 ET. Its options are labeled 15-minute delayed. These provider statistics are not executable premiums or a measured post-earnings volatility decline; check live prices before trading. [13]
For the wheel, a cash-secured put means reserving money to buy assigned shares; a later covered call does not erase their downside. A covered call also caps participation in another rally. The decision is about accepting those obligations, not simply collecting a premium after good news. Current spreads, contract terms and live prices still need checking. [8][9][10]
Arithmetic only—not a quoted setup
For a hypothetical standard 100-share $1,300 put, assume a $10-per-share premium: $1,000 credit, $130,000 gross assignment cash and a $1,290 breakeven. Maximum loss if the stock becomes worthless is $129,000 before fees. A hypothetical $1,400 covered call could require selling the covered shares at $1,400 despite a larger rally. No actual premium, expiry or recommended strike is claimed. [8][9][10]
The main risk
Rapid unit growth can coexist with lower prices, nonrecurring revenue benefits and expensive research acquisitions. A short put cannot insure against a gap lower, and writing calls against assigned shares cannot guarantee recovery of the purchase cost. Premium income is limited; stock downside remains substantial. [1][8][9]
What to watch next
- November 4: Novo Nordisk’s confirmed results date; its call is scheduled for 07:00–08:00 EST. Competitor commentary may change expectations even after Lilly’s own earnings. [5]
- December 9: the decision day of the Fed’s December 8–9 meeting. Longer-dated contracts can span this separate macro event. [7]
- For the next Lilly update: monitor realized pricing and the conversion of treatment demand into sales. No next Lilly earnings date is assumed from an unconfirmed calendar.
- For price monitoring: $1,288.30 is the prior close and $1,330.00–$1,408.38 the saved intraday range, not guaranteed support or resistance. [2]
